 Middle School Field Hockey Information Letter

 

Welcome Field Hockey parents,

 

The following are lists of necessary equipment and forms.

Equipment:

 

• Mouth guard (more than one) must be colored (no clear or white)

• Goggles - please see the back of this sheet for goggle specifications

• Shin guards

• Black AND white socks

• Black AND white t-shirt or compression shirt to wear during cooler weather under jerseys

• Cleats for games and sneakers for practice

• Stick (can be borrowed if you prefer not to purchase one)

• Black skort (we have a limited supply for those who need to borrow one)

• Game jerseys will be provided

 

Forms (see MVMS website):

 

• Copy of updated physical (9-6-2021 or more recent), can be uploaded to FamilyID or sent in to main office if you are unable to upload

• Forms are completed on line this year at FamilyID
